Integrated Community Wellness

Integrated Community Wellness Model:

Collective Souls Holding Space (CSHS) delivers services through an Integrated Community Wellness Model designed to delivering prevention, crisis response, healing, and long-term stability across the lifespan. 

Wellness, Counseling, and Crisis Response

Trauma-informed counseling, peer support, and immediate crisis response following violence, loss, or emergencies. Services include hospital accompaniment, emotional support, and ongoing care coordination. CSHS also provides specialized wellness support for first responders, correctional officers, National Guard members, and veterans, addressing trauma, burnout, and secondary stress.

Violence Prevention and Community Healing:

Community-based violence interruption initiatives, trauma response teams, and healing-centered interventions that promote safety, accountability, and connection. Activities include peace ceremonies, Peace Pole installations, community healing events, and relationship-based de-escalation efforts informed by public health and trauma-recovery models.

Youth and Alternative Education:

Positive Youth Development programming that builds social-emotional skills, leadership, accountability, and resilience. CSHS offers alternative education opportunities for youth and adults, service learning experiences, and mentorship to support engagement, confidence, and long-term success.

Reentry and Transitional Support:

Wraparound support for formerly incarcerated individuals and their families, including court accompaniment, systems navigation, workforce readiness, and community reintegration. Services focus on stability, dignity, and reducing barriers to successful reentry.

Housing Hope & Sustainability Program:

A trauma-informed initiative supporting individuals and families experiencing housing instability. HHSP provides wellness support, housing navigation and stabilization, cultural and language access services, and community connection to promote long-term housing security and wellbeing.

Community Wellness Center:

A trusted, holistic healing space that integrates behavioral health counseling, peer advocacy, wellness practices, food assistance, emergency support, and family services. The Wellness Center serves as a hub for prevention, healing, and community connection.

Early Childhood Education Programming:

CSHS’s Integrated Community Wellness Model emphasizes early intervention and prevention, recognizing that lifelong wellbeing begins in early childhood. Through Simone’s Bean Pod, children ages 0–5 engage in play-based, trauma-aware learning that supports emotional regulation, cognitive development, problem-solving, and social-emotional growth. Families are supported alongside children, strengthening protective factors and school readiness. This early childhood work is embedded within a broader continuum that includes family wellness, youth development, housing stability, and community healing—ensuring children grow up in safer, more connected environments that support long-term educational and developmental outcomes.
